Every individual and family has different circumstances, priorities and goals. Services are personalized and may be provided for a specific need, during a significant transition or as ongoing support over time.

Care Planning & Coordination
 
A clear, personalized plan can make a complicated situation feel more manageable. We work with you to understand your needs, identify priorities and coordinate the people and services involved in your support.
 
Support may include:

Family Caregiver Support

✓   Providing updates to authorized family members

✓ Communicating with providers and community services, with consent

Life Transition & Resilience Coaching
 

Significant life changes can affect identity, routines, relationships and well-being. We provide personalized coaching and emotional support to help individuals recognize their strengths, clarify their goals and move forward at their own pace.

Clear information, informed consent and professional accountability are important parts of working together.

Professional Scope
 
Services are provided by Karen Davis, RSW, in accordance with the Alberta College of Social Workers Code of Ethics and Standards of Practice.
 
Aging Well provides non-clinical professional social work services, including care navigation, advocacy, coordination, caregiver support, emotional and resilience support, and life transition coaching. These services complement—but do not replace—medical, nursing, psychological, legal, financial or emergency services.
 
Aging Well does not provide medical diagnosis or treatment, nursing or personal care, medication management, legal or financial advice, personal transportation, psychotherapy, or emergency and crisis intervention.
 
When needs fall outside our professional scope, we will help identify and connect clients with appropriate healthcare professionals, community agencies or specialized services whenever possible.
 

 


Client Handbook
 
Before services begin, clients receive the Aging Well Care Navigation Client Handbook. It explains what you can expect when working with us and helps ensure that you understand your rights, responsibilities and available services.
 
The Client Handbook includes information about:
 

Our services and professional boundaries
Client rights and responsibilities
Privacy, confidentiality and informed consent
Communication and virtual services
Scheduling, fees and payment
Transportation coordination and client expenses
Safety, respectful conduct and ending services
Questions, feedback and the complaints process

You will have an opportunity to review the handbook, ask questions and provide informed consent before ongoing services begin.

An older adult is preparing to return home after hospitalization. Aging Well helps organize discharge information, identify follow-up needs, coordinate community supports and create a clear transition plan.

Supporting a Family

from a Distance

An adult child living outside Calgary is concerned about a parent’s changing needs. Aging Well provides local care coordination, attends appointments when requested and shares updates with authorized family members.

Exploring Housing & Community Supports

An older adult is considering whether their current home continues to meet their needs. Aging Well helps explore living arrangements, community services, transportation options and supports that promote independence and well-being.

Managing Multiple Appointments

​An individual is seeing several healthcare providers and feels overwhelmed by appointments and recommendations. Aging Well helps organize information, prepare questions, document next steps and coordinate communication with consent.

Navigating Caregiving Decisions

A family caregiver is balancing work, family responsibilities and increasing care needs. Aging Well helps clarify priorities, explore available resources, facilitate family communication and develop a manageable plan.

Moving Through a Life Transition

An individual is adjusting to retirement, loss, relocation or a change in health. Aging Well provides life transition and resilience coaching to help identify strengths, establish meaningful goals and create practical next steps.
