Karen Davis, RSW, Founder & Care Navigator

Meet Karen Davis, RSW
Supporting individuals and families through compassionate care navigation, advocacy and life’s transitions.
When medical care ends, support continues.

Professional Experience. Personal Understanding. Compassionate Guidance.
I’m Karen Davis, RSW, Founder & Care Navigator of Aging Well Care Navigation Ltd. As a Registered Social Worker with more than 25 years of experience, I have supported individuals, families and communities through healthcare, human services, leadership, advocacy and life’s many transitions.
Throughout my career, I have worked in frontline and leadership roles across healthcare, non-profit, post-secondary and community-based settings. I have helped individuals navigate complex systems, supported families facing difficult decisions and worked alongside healthcare providers and community organizations to coordinate meaningful support.
My professional experience has shaped my knowledge, while my personal experiences supporting aging family members have shaped my understanding of the emotional and practical realities families face. I know how quickly appointments, changing health needs, family responsibilities and important decisions can become overwhelming.
I created Aging Well to help connect the pieces—providing compassionate care navigation, advocacy, care coordination, caregiver support, and life transition and resilience coaching so individuals and families can feel informed, supported and more confident about the path ahead.
You do not have to navigate aging, caregiving or life’s transitions alone.

Why I Started Aging Well
Throughout my career and personal life, I have seen how quickly individuals and families can become overwhelmed when health, caregiving responsibilities or living circumstances begin to change.
Healthcare appointments are often brief, services may be difficult to access or coordinate, and families are frequently left trying to understand information, explore options and make important decisions while managing their own responsibilities and emotions. Although many valuable services exist, connecting all the pieces can be challenging.
I founded Aging Well Care Navigation to help bridge the gaps between healthcare, community resources, daily living and family support. My goal is to provide a consistent source of professional guidance so individuals and families feel heard, informed and supported as they make decisions and move through life’s transitions.
Aging Well was founded on a simple belief: no one should have to navigate aging, caregiving or significant life transitions alone.
